Lost or Stolen Passport
If your still valid passport was lost or stolen, you must report it as invalidated and apply for a replacement. Passports reported lost or stolen are invalidated and can no longer be used, even if found.
Requirements
- Application: Complete online form DS-64 to report the loss and form DS-11 to apply for a new passport (do not sign yet).
- Photos: Provide one 2’’x2’’ photo.(Can be taken in office)
- Proof of Citizenship: Submit an original or certified copy of your birth certificate, naturalization certificate, or previously issued U.S. passport.
- Travel Itinerary: Include a copy of your travel itinerary or airline ticket.
- Proof of Identity: A copy of your state ID or driver’s license.
- Authorization Letter: Authorization form.
- Government Fees: Pay the appropriate fees via check or money order:
- For an adult passport book only: $211.36 payable to the U.S. Department of State.
- For an adult passport book and card: $241.36 payable to the U.S. Department of State.
- For a minor passport book only: $181.36 payable to the U.S. Department of State.
- For a minor passport book and card: $196.36 payable to the U.S. Department of State.
- $35.00 payable to the clerk of the acceptance agency.
Deliver the sealed envelope from the clerk to our office or call for instructions.

Remarks: For overnight delivery, include a prepaid FedEx air bill or pay $24.00 to Passport & Visa Solutions.
